Thank you for choosing Bluewater Day Camp! This form is required to be completed after online registration is made to confirm the camper's spot. Forms are due 5 days after online registration.  Please visit ACTIVENET to register for camp.

Personal information is collected by the authority of the Municipal Act, 2001, S.O. 2001, c.25 for the administration of the Municipality of Bluewater's day camp program. Use and disclosure of information is made in accordance with the Municipal Freedom of Information and Protection of Privacy Act. For campers attending Weeks 1 (July 7 to 11) and 4 (July 28 to Aug.1)

We plan to go swimming during Weeks 1 and 4. Select which option best describes your child's swimming ability.
