Dog Tag Application Form

All dogs must be registered each year by February 28 and within 15 days of taking possession of a dog. A dog tag is proof of registration and a form of identification should the dog be lost. After February 28, a late fee will be added to the tag fee for each dog. Failure to Register a Dog for the current year may result in a charge under the Provincial Offences Act of $250.00 for each offence.

Households are not permitted to own more than three (3) dogs unless eligible for a kennel licence. Please inform the Municipality if you no longer have a dog.

Please fill out this dog tag application form and submit. A dog tag will be mailed to you after your payment is received. Acceptable methods of payment are cash, cheque or debit to the Municipality of Bluewater, 14 Mill Avenue, Zurich ON N0M 2T0
